GAP NURSING
Gap nursing: filling in where home health agencies, physician offices and family caregiving cannot. Home nursing agencies are a fantastic alternative to in-facility care. However, there are times when supplemental or additional care is required for optimal outcomes for your loved one. Gap care can serve as a “fill-in” when agency is not available but can also take the form of primary nursing care in the home.

CARE COORDINATION
Care Coordination: helping families communicate with physicians, specialists, therapists, pharmacies and healthcare providers. Taking the lead in assisting patients and family navigate a sometimes-complex healthcare system.

PATIENT & FAMILY EDUCATION
Patient and Family Education: Paula takes the time needed to help patient and family understand their condition, their treatment and potential outcomes. Ongoing supportive education as situations change, answering patient questions with honesty and compassion.

PATIENT ADVOCACY
Patient Advocacy: Often times we feel unheard or misunderstood in the medical community. Having someone who is familiar with the language and inner workings of the system to support your journey is crucial. This service includes your personal advocate, Paula, to accompany you or your loved during Emergency Room or Urgent Care Visits, as well as scheduled MD appointments if needed.
